For your child to participate in the sport camps at Edgewood High School, the undersigned parent/guardian agrees to release and hold harmless Edgewood High School, its representatives, agents, and employees from all liability for any claims, including negligence, resulting from participation in the program the participant is registering for. As the parent or legal guardian of the participant, I authorize my child to participate in this program. I am responsible for any illness or injuries resulting from participation in this program. As the parent/guardian of the participant, you recognize there are inherent dangers associated with any Edgewood program and that participation could result in being exposed to such dangers and hereby assumes full responsibility for any risk of bodily injury, death, or property damage arising out of or related to the event. The undersigned also certifies that the registered participant is in good health and has no physical impairment, injury, or illness that will make participation dangerous to himself/herself or others. In addition, as the parent/guardian, you waive your right to bargain over the terms of this liability/assumption of risk agreement waiver. In a medical emergency during the program, I authorize representatives of Edgewood High School to obtain medical treatment for the registered participant. I want you to know that I will be responsible for any medical and related bills incurred on behalf of the participant for any illness or injury that the participant may sustain related to or during the athletic camp. As the parent/guardian, you have read this release and waiver of liability and assumption of risk agreement, fully understand its terms, and have signed it freely and voluntarily to complete the registration process for the program participant.
